New forms effective February 19, 2020 are available on the US Courts website.

Most Official Bankruptcy Forms and Director’s Forms will be replaced with substantially revised, reformatted and renumbered versions effective December 1, 2015. Click here for a presentation of the new forms and filing instructions.

In addition to the changes to the bankruptcy forms, Federal Rule of Bankruptcy Procedure 1007 and Federal Rules of Civil Procedure 1, 4, 16, 26, 30, 31, 33, 34, 37 and 55 have been amended and are scheduled to be effective on December 1, 2015.
